Hospice of the Valley Unites with Sutter Care at Home

Hospice of the Valley (HOV) and Sutter Care at Home (SCAH) have joined to continue providing the legacy of high-quality programs and services dedicated to those with serious illness or needing end-of life care. This collaboration enhances Hospice of the Valley’s ability to provide hospice care and grief support services in the greater Santa Clara County. Hospice of the Valley celebrates our more than 40 years of service and continues to serve its community with the same not-for-profit mission and values with our integration to Sutter Care at Home.

Hospice of the Valley – when it matters the most. Since 1979 over 23,000 of our friends and neighbors have received compassionate end-of-life care and grief support from Hospice of the Valley, the oldest community-based non-profit hospice in Santa Clara County. Trusted by physicians, patients, and families, we are neighbors serving neighbors.

Hospice care is a philosophy of care as well as a comprehensive set of services as life becomes more limited. First, as a philosophy of care, hospice provides pain and symptom management when cure is no longer an option. Our goals are to honor the wishes of our patients, to redefine hope and support quality of life whose hallmark is peace, to honor the dignity of each patient, and to support the patient and family through their grief process.

Second, as a set of services, hospice care, like a safety net, surrounds patients and their families with experts in the fields of pain and symptom management, in physical, emotional and psychological care, spiritual support, and grief support. Our hospice care team collaborates closely with the patient’s own physician to create a plan of care which is driven by the needs and wishes of the patient, family and loved ones.

Hospice of the Valley also offers grief support at no-charge for the family and loved ones of the patient for 13 months as part of our hospice services. Our Community Grief Center also offers bereavement support to grieving community members on a sliding scale fee.

Hospice of the Valley is now a division of Sutter Care at Home.
