Are you the parent of a student between 12 and 22 years old? Join us for a comprehensive workshop on navigating the Individualized Transition Plan (ITP) to ensure your student is on the path to a successful transition to adulthood. The ITP is a crucial part of the Individualized Education Program (IEP) and outlines the transition services and goals necessary for your student’s future success.
Please join Luisamaria Castillo, TASK’s (PHP’s sister agency in Southern CA) San Diego/Imperial Valley Area Manager, as she discusses:
– What Are Transition Services Under the Individuals with Disabilities Education Act (IDEA)? Understanding how the law supports your student’s transition journey
– What Is an ITP? Explaining key components (transition assessments, course of study, diploma, new pathway to diploma, certificate of completion, “”age of majority””) of an Individualized Transition Plan
– Goals and Services for Transition Under IDEA: Setting achievable transition goals and identifying meaningful support services
– Who Is Involved in Transition Planning – and How Can Parents Help Drive Success? Discovering the roles of parents, educators, and outside agencies
– Fostering Self-Advocacy Skills: Encouraging your student to become their own advocate and build independence
Parents and caregivers are essential partners in designing a well-rounded ITP, helping young adults achieve independence, employment, and further education. Don’t miss this opportunity to learn how to advocate for your student’s future!
Accommodations: If you need accommodations to participate in the training or assistance with registration, please contact us as soon as possible at [email protected]. We typically require 10 days to arrange accommodations to ensure your inclusion.
This is a Connections California event, a project of Parents Helping Parents – Silicon Valley, in collaboration with the Team of Advocates for Special Kids (TASK). Connections California is sponsored in part by the San Andreas Regional Center and the California Department of Developmental Services.
